Does obicetrapib lower Alzheimer's risk in APOE4 carriers, and when will it be available?

Obicetrapib lowers LDL, ApoB and Lp(a), and in one trial substudy it may have slowed Alzheimer's blood markers, most of all in the small APOE4/4 group. That is a biomarker signal from one program, not proof that it prevents memory loss or heart attacks; those trials are still running. Ask your doctor about its approval status and any trial you could join.

What the research says

Grade C

APOE4-specific evidence: yes

Obicetrapib lowers LDL, ApoB and Lp(a) (high certainty) and may slow Alzheimer's blood markers in APOE4 carriers (low certainty, one substudy); it is not yet shown to prevent dementia or heart attacks.

Key studies

  • Nicholls 2025, N Engl J Med (BROADWAY)Grade A

Phase 3 trial in high-risk patients already on maximum lipid therapy: obicetrapib cut LDL by about a third over a year, with side effects similar to placebo.

  • Araujo 2025, Am J Prev CardiolGrade A

7 trials, 3,381 people: LDL down about 37%, Lp(a) about 37%, ApoB about 25%; new-onset diabetes 12% lower; no excess side effects.

  • Davidson 2026, J Prev Alzheimers Dis (BROADWAY substudy)Grade C

1,535 heart patients: obicetrapib slowed the rise in blood p-tau217 over 12 months; in the small APOE4/4 group p-tau217 fell 7.8% versus a 12.7% rise on placebo.

  • Schmidt 2024, Alzheimers Res Ther

Genetic (Mendelian randomization) study: naturally lower CETP went with less heart disease and lower Lewy body dementia risk.

  • Nicholls 2025, J Am Coll CardiolGrade C

Pooled, exploratory analysis of two trials: fewer coronary events on obicetrapib, appearing after 6 months; outcome trial still needed.

  • Barter 2007, N Engl J Med (ILLUMINATE)

An earlier CETP drug, torcetrapib, raised blood pressure and deaths; videos attribute this to the molecule, not the target.
