Do GLP-1 drugs like semaglutide protect an APOE4 brain?

Semaglutide lowers weight, blood sugar and inflammation; that is well proven. In two large trials in early Alzheimer's, about six in ten of them carriers, it did not slow decline. People with diabetes who take GLP-1 drugs get dementia less often, so they may help prevent it, but no trial has tested prevention yet.

What the research says

Grade C

APOE4-specific evidence: none

GLP-1 drugs clearly improve weight and blood sugar and did not slow early Alzheimer's in two large trials; lower dementia rates in users with diabetes are a promising but unproven signal for prevention.

3,808 people with early Alzheimer's: oral semaglutide did not slow decline on CDR-SB over two years.

  • Tang 2025, JAMA NeurologyGrade C

Target trial emulation in about 34,000 people with type 2 diabetes: GLP-1 starters had a third lower dementia rate (observational).

Meta-analysis of heart and diabetes trials: GLP-1 drugs went with fewer dementia diagnoses (not a primary trial outcome).

  1. 3:35The Ozempic Alzheimer's Trial Failed. Why That's Good News ·

Oral semaglutide did not slow early Alzheimer's.
