APOE4 question

I just found out I carry APOE4: what should I do first, and what matters most?

Start with the basics, not a supplement stack: regular exercise, a Mediterranean-style diet, good sleep, and a baseline blood test so you know your numbers. Programmes that combine these habits with blood pressure, sugar and cholesterol checks probably slow decline in thinking a little, and carriers did as well as non-carriers in the big trials. Change one or two things at a time, find people who understand, and bring your doctor in for anything with side effects.

This is not medical advice. Phoenix educates. Your doctor decides what is right for you.

What the research and Kevin's videos say

What the research says

Grade B

APOE4-specific evidence: limited

Large trials found that combining exercise, healthy eating, brain training and heart-health checks probably slows decline in thinking slightly, with carriers doing as well as others; no trial has tested which step to take first.

Key studies

  • Ngandu 2015, LancetGrade B

FINGER: in 1,260 at-risk older adults, a two-year programme of diet, exercise, brain training and vascular checks improved overall thinking scores versus general health advice.

  • Solomon 2018, JAMA NeurolGrade B

FINGER by APOE: carriers benefited from the programme, but the difference from non-carriers was not statistically significant.

  • Baker 2025, JAMAGrade B

U.S. POINTER, 2,111 adults aged 60-79: a structured programme improved thinking scores slightly more than a self-guided one, and carriers benefited as much as non-carriers.

2 more key studies
  • Hughes 2020, JAMAGrade B

Meta-analysis of 12 blood-pressure trials (92,135 people): lowering blood pressure went with fewer cases of dementia or cognitive impairment.

  • Lourida 2019, JAMAGrade B

UK Biobank, about 196,000 adults: a healthy lifestyle went with lower dementia risk even among people at high genetic risk (observational).
