NCT05515224 · RECRUITING

5-Cog 2.0: A Pragmatic Clinical Trial

This trial tests whether a quick memory and thinking check, called the 5-Cog, helps primary care doctors spot mild cognitive impairment and dementia more often than usual care. It takes under five minutes. About 6,600 older adults with memory concerns are involved across 22 clinics. It is a care-delivery study, not a drug trial.

You may qualify if

  • 65 years and older
  • Presence of cognitive concerns
  • English or Spanish speaking.
  • Able to see and hear well enough to complete assessments.

You're excluded if

  • Prior diagnosis of dementia (documented in the electronic medical record or reported by physicians).
  • Permanent nursing facility residents.

The sponsor's own eligibility wording, lightly reformatted. The study team makes the final eligibility decision — worth discussing with your doctor.
