NCT07889830 · NOT YET RECRUITING
Inpatient Hospice Transitions Checklist
This study tests a checklist built into the Epic hospital record system. It helps hospital teams decide if a person with advanced dementia may be eligible for hospice, and guides conversations with family decision-makers. Researchers are measuring whether it is practical to use. It is an early feasibility study, not a test of whether it improves outcomes.
